5 and under:
-fell face-first into a coffee table and split the piece of skin in the top part of your mouth that connects your lip to your gum. i don't really have one.
-flipped my dad's motorcycle on top of me from getting on the wrong side.
10 and under:
-stepped on a really long rusty nail that went through my shoe and a little into my foot on vacation in n.c.
-fell down in my bathroom and caught about 2 inches of skin on my knee on a sharp corner of a cabinet and proceeded to bleed all over my bathroom
-was playing football with my friend and his older cousins (they were about 14) and i caught the ball...so they tackled me into the ground and smashed my glasses into the bridge of my nose and i bled everywhere
11 - 16:
-nothing i can remember
17 / 18:
-fell off a school bus and skinned my knees / palms
-playing red rover, i ran into this kid to break their line and...well i didn't, and tripped and bashed the back of my head into lawn rocks
-3 days before prom i was trying to drive my friend's scooter and wiped out in the parking lot of the church i was taking my finals at, and scraped my left arm up (gg prom dress)
19 / 20:
-tripped down stairs at my apartment in orlando and skinned my knees
21 (this past summer):
-my bf's aunt and uncle have a lake house in georgia, and i tripped off their front porch and fell onto their concrete driveway. at first i didn't realize how bad it was, but then i noticed that there was blood running down my leg and had started to coagulate, so we ran in the house to see wtf happened. my knee and a good portion of my shin were skinned and swollen. boyfriend had to patch me up, and then i hobbled around for a few days. it was horrible when we had to swap bandages and pour peroxide on my knee. hurt like a bitch.
that's all i can think of.

I've only had one real severe injury.
I was in grade 1 (December '96, I distinctly remember) and it was snowing for the first time. So like an idiot I ran around at recess trying to catch snow with my tongue.
And then he hit me.
There was a basketball game going on at the time and this huge grade 8 guy, without noticing, ran into me hard, and so I went face first into the pavement. I lost maybe 3-5 teeth, face was bloody/cut up. I remember the whole time I was crying for a band-aid (lolstupid). I was at the water fountain trying to wash out the blood and I saw my own teeth just come out...needless to say it was pretty scary.
Stayed home for 2 weeks, always had a scarf covering most of my face to hide the dried skin when going to the doctors. It was the sole reason why I needed braces. Otherwise I actually don't get injured at all/often.

it didn't even really hurt, i stayed in the rest of the inning and even turned a double play, the only reason i had to come out afterward was cause it had swollen so much i couldn't put my glove back on
also one time i burnt my arm on an oven door that left a weird huge triangle shaped scar but that didn't really hurt either
guess i'm just lucky

That's about it as far as injuring myself, however, I almost broke a kid's collarbone (I might have, honestly) playing touch football. The kid was the scrawniest person in our entire grade and I was kinda chubby at that point. One of the guys who was a total asshole and got way too worked up over recess game pushed me into him so I wouldn't intercept the pass. I ended up tripping him and landed right on top of him. I felt like a total dick even though it wasn't my fault

I know right. The main downside to that is I can barely use my right arm and my memory is pretty shot. You gotta keep in mind that I fell into a bunch of sand to break the fall for the most part.

Better your right arm than you being six feet under, though.
